Are you sure you're even trying to talk to the disk? What happens if you just connect the USB interface with no drive present? Do you still get a /dev/sdb entry? Yes, the /dev/sdb is still shown with the same 0 bytes size. So I am suspicious of the USB->IDE adapter's ability to read these old C/H/S...

On the Integra-B I had to select a graphics mode, then run */CODE and it fills the screen with the program and then the music starts and the mode switches. If I just boot the disc, I get a plain blue screen most of the time and if it does run it gets stuck when it is supposed to show the BEEP letter...

Can you put up a close up photo of the soldering around the CPLD? There may be a solder bridges between pin and/or pins from the CPLD may not be soldered to the PCB pads.
